Retinitis Pigmentosa: Symptoms, Causes, Diagnosis, and Treatment

Retinitis pigmentosa, often shortened to RP, is a group of inherited eye diseases that gradually damage the retina—the light-sensitive tissue lining the back of the eye.
Unlike many eye conditions that affect sharp central vision first, retinitis pigmentosa often begins by affecting night vision and peripheral vision. Over time, the field of vision can become progressively narrower, sometimes producing what patients describe as “tunnel vision.”
There is currently no single cure for all forms of retinitis pigmentosa. However, advances in genetic testing, retinal imaging, low-vision rehabilitation, and gene therapy have dramatically changed how doctors evaluate and manage inherited retinal diseases.

What Is Retinitis Pigmentosa?
Retinitis pigmentosa is not actually one single disease. It is a large group of genetically inherited retinal disorders that cause progressive degeneration of the retina's photoreceptor cells.
The retina contains two major types of photoreceptors:
Rods help us see in dim lighting and are especially important for peripheral vision.
Cones provide sharp central vision, color vision, and detailed daytime vision.
In many forms of retinitis pigmentosa, the rods are affected first. This explains why one of the earliest symptoms is often difficulty seeing at night.
As the disease progresses, cone cells may also become involved, potentially affecting central vision, reading, color perception, and facial recognition.
The National Eye Institute describes RP as a group of rare genetic eye diseases in which retinal cells gradually break down, producing progressive vision loss.
What Are the First Symptoms of Retinitis Pigmentosa?
Symptoms and their age of onset can vary significantly depending on the specific genetic mutation involved.
1. Difficulty Seeing at Night
Night blindness, or nyctalopia, is one of the classic early symptoms of RP.
Someone may notice difficulty:
Walking through a dark restaurant
Driving at night
Seeing after entering a dark movie theater
Navigating a dim hallway
Adjusting when moving from bright sunlight into a darker room
Children may appear unusually uncomfortable or hesitant in dark environments.
2. Loss of Peripheral Vision
As RP progresses, peripheral or side vision may gradually decrease.
A person may begin:
Bumping into objects
Missing people approaching from the side
Having difficulty navigating crowded areas
Struggling with sports or activities requiring peripheral awareness
Eventually, the remaining visual field may become quite narrow, producing tunnel vision.
3. Difficulty Adjusting Between Light and Dark
Because the rod photoreceptors are involved in dark adaptation, patients may take significantly longer than expected to adjust to changes in illumination.
4. Light Sensitivity
Some people with RP experience increased sensitivity to bright light, known as photophobia.
5. Reduced Color or Central Vision
Central vision often remains relatively preserved during the earlier stages of many forms of RP.
However, as cone cells become affected, patients can develop:
Reduced reading vision
Difficulty recognizing faces
Reduced color discrimination
Decreased visual acuity
The speed of progression varies dramatically from one person to another.
What Causes Retinitis Pigmentosa?
Most cases of retinitis pigmentosa are caused by mutations in genes responsible for maintaining healthy retinal cells.
Hundreds of genetic variations have been associated with inherited retinal diseases, and RP can follow several inheritance patterns.
These include:
Autosomal Dominant
Only one altered copy of a gene may be necessary for the condition to occur.
An affected parent may have a chance of passing the condition to a child.
Autosomal Recessive
A person generally inherits one abnormal copy of the gene from each parent.
The parents may carry the mutation without having symptoms themselves.
X-Linked Retinitis Pigmentosa
The altered gene is located on the X chromosome.
X-linked RP may produce significant disease, particularly in affected males.
Sporadic Cases
Sometimes a patient is diagnosed with retinitis pigmentosa even though there is no known family history.
This does not necessarily mean the disease is not genetic. Family members may carry mutations without obvious symptoms, or a genetic change may have occurred without a previously recognized family history.
Because of the number of genes associated with RP, genetic testing and genetic counseling are increasingly important parts of the evaluation.
What Does Retinitis Pigmentosa Look Like During an Eye Exam?
As the disease progresses, an eye doctor may see several characteristic changes inside the eye.
One of the classic findings is called bone-spicule pigmentation.
These are dark, irregular deposits of pigment that develop in the peripheral retina and can create a pattern somewhat resembling small bone fragments.
Other classic findings may include:
Narrowing of the retinal blood vessels
A pale or waxy appearance of the optic nerve
Retinal pigment changes
Thinning or loss of retinal tissue
However, these findings may be subtle or even absent during earlier stages.
That is why additional retinal testing can be extremely important.
How Is Retinitis Pigmentosa Diagnosed?
Diagnosing RP usually involves more than simply checking whether someone can read the eye chart.
A comprehensive evaluation may include several different tests.
Dilated Retinal Examination
Dilating the pupils allows the optometrist or ophthalmologist to carefully evaluate the retina and optic nerve.
Doctors look for the characteristic retinal changes associated with inherited retinal degeneration.
Visual Field Testing
Visual field testing maps how much peripheral vision remains.
Someone with RP may develop a ring-shaped area of field loss that eventually progresses toward more extensive peripheral vision loss.
Visual field testing is also useful for monitoring changes over time.
Optical Coherence Tomography — OCT
OCT produces extremely detailed cross-sectional images of the retina.
In RP, OCT may demonstrate changes or loss involving the photoreceptor layers.
It can also identify complications such as cystoid macular edema, which can reduce central vision but may sometimes be treatable.
Fundus Autofluorescence — FAF
Fundus autofluorescence imaging provides information about the health and metabolism of the retinal pigment epithelium.
It can reveal patterns of retinal stress and degeneration that may not be obvious during a standard examination.
Electroretinography — ERG
An electroretinogram, or ERG, measures the retina's electrical response to light.
Because RP affects photoreceptor function, ERG responses can become reduced and may eventually become difficult to detect in advanced disease.
ERG testing can be especially valuable when confirming or characterizing an inherited retinal degeneration.
Genetic Testing
Genetic testing attempts to identify the specific mutation responsible for the retinal disease.
This has become increasingly important because it may:
Confirm the diagnosis
Clarify the inheritance pattern
Provide information for family members
Help estimate how the disease may behave
Determine eligibility for certain clinical trials
Identify patients who may qualify for gene-specific therapy
Genetic counseling may also help patients understand what the results mean for themselves and their families.
Is There a Cure for Retinitis Pigmentosa?
There is currently no universal cure for retinitis pigmentosa.
Because many different genes can cause RP, a treatment that works for one genetic form of the disease may not work for another.
However, inherited retinal disease is one of the most rapidly advancing areas of ophthalmic research.
Research is investigating approaches such as:
Gene replacement therapy
Gene editing
RNA-based treatments
Neuroprotective medications
Stem-cell approaches
Photoreceptor restoration
Optogenetics
Retinal prosthetic technologies
These approaches are designed either to preserve remaining retinal cells or potentially restore some visual function.
Gene Therapy for Certain Inherited Retinal Diseases
Gene therapy has already moved from the laboratory into clinical medicine.
Luxturna® (voretigene neparvovec-rzyl) is an FDA-approved gene therapy for patients with confirmed mutations affecting both copies of the RPE65 gene who have viable retinal cells.
RPE65-associated disease represents only a small percentage of inherited retinal dystrophies, so Luxturna is not a general treatment for everyone with RP.
But its development was historically important because it demonstrated that replacing a defective gene in retinal cells could improve visual function in a human inherited retinal disease.
Researchers are now investigating gene-based treatments for numerous additional inherited retinal disorders.
Can Vitamins Treat Retinitis Pigmentosa?
Patients may encounter recommendations online involving vitamin A, lutein, DHA, fish oil, or other supplements.
Historically, high-dose vitamin A was studied as a possible way to slow certain forms of RP. However, its role remains controversial, and high doses of vitamin A can produce significant side effects, including liver toxicity.
Because RP represents many genetically different diseases, a supplement that might theoretically benefit one condition could be ineffective—or potentially inappropriate—for another.
Patients should not begin high-dose vitamin A or other RP-specific supplements without discussing them with an inherited-retinal-disease specialist or eye doctor.
Other Eye Problems Associated With Retinitis Pigmentosa
Patients with RP can develop other eye conditions that may further reduce vision.
These include:
Cataracts
Certain types of cataracts occur more frequently in people with RP.
If the cataract becomes visually significant, cataract surgery may sometimes improve the vision that remains.
Cystoid Macular Edema
Fluid may accumulate within the central retina or macula.
This can reduce central vision and may be detected with OCT imaging.
Unlike the underlying retinal degeneration, macular edema may sometimes respond to treatment.
Refractive Error
Patients may still benefit from updated glasses or contact lenses.
Correcting myopia, hyperopia, or astigmatism does not treat RP itself, but it can help maximize the visual function that remains.
The National Eye Institute recommends regular examinations because conditions including cataracts, refractive errors, and cystoid macular edema may occur alongside RP.
Living With Retinitis Pigmentosa
A diagnosis of retinitis pigmentosa does not automatically mean someone will suddenly become blind.
RP usually progresses gradually, and the severity and rate of progression vary considerably depending on the genetic subtype.
Many people maintain useful central vision for years or decades.
As vision changes, low-vision rehabilitation can help patients continue working, reading, traveling, using computers, and remaining independent.
Helpful technologies may include:
Magnification devices
Electronic reading systems
Screen readers
High-contrast displays
Smartphone accessibility features
Orientation and mobility training
Specialized lighting
Telescopic devices
Wearable assistive technology
The goal is not simply to measure how much vision has been lost. It is to help patients make the best possible use of the vision they still have.
Should Family Members Be Examined?
Because RP is often inherited, an eye doctor may recommend that certain relatives receive eye examinations or genetic counseling.
Genetic testing may sometimes identify family members who carry the same mutation even before significant symptoms appear.
Understanding the inheritance pattern can also be useful for patients who are planning families.
When Should You See an Eye Doctor?
Schedule a comprehensive eye examination if you notice:
Increasing difficulty seeing at night
Unexplained loss of peripheral vision
Frequent bumping into objects
Difficulty adjusting from bright to dark environments
Unexplained progressive vision changes
A family history of retinitis pigmentosa or inherited retinal disease
These symptoms do not necessarily mean you have retinitis pigmentosa. Cataracts, glaucoma, retinal disease, neurological conditions, and other eye disorders can sometimes cause similar symptoms.
