Glaucoma is one of the most important eye conditions to detect early because it can slowly damage the optic nerve before a patient notices symptoms. The optic nerve is the cable that carries visual information from the eye to the brain, and once glaucoma causes vision loss, that damage is usually permanent.
The good news is that glaucoma can often be monitored and treated when it is found early. That is why routine eye exams, eye pressure checks, optic nerve evaluation, OCT imaging, and visual field testing are so important.

What Is Glaucoma?
Glaucoma is a group of eye diseases that damage the optic nerve. Many patients associate glaucoma with “high eye pressure,” and elevated eye pressure is an important risk factor. However, glaucoma is not just about pressure. Some patients can develop glaucoma even when their eye pressure is in a normal range. The key concern is whether the optic nerve is being damaged over time.
The most common type is open-angle glaucoma, which usually develops slowly and painlessly. Because it often has no warning signs in the early stages, many people do not know they have it until vision loss has already started.
Why Glaucoma Is Called the “Silent Thief of Sight”
Glaucoma is often called the silent thief of sight because early glaucoma usually does not cause pain, redness, or obvious blurry vision. In many cases, central vision stays clear while peripheral, or side, vision is slowly affected first.
This means a patient may still read, drive, and see straight ahead normally while glaucoma is quietly damaging the optic nerve. By the time vision changes become noticeable, the disease may already be more advanced.

Common Risk Factors for Glaucoma
Anyone can develop glaucoma, but some people have a higher risk. Risk factors may include:
Family history of glaucoma
Age over 40, with risk increasing as you get older
Elevated eye pressure
Thin corneas
Diabetes, high blood pressure, or vascular disease
History of eye injury or eye surgery
High nearsightedness
African American, Hispanic/Latino, or Asian ancestry, depending on glaucoma type
Long-term steroid medication use
The National Eye Institute recommends that people at higher risk have comprehensive dilated eye exams every 1 to 2 years.
What Symptoms Can Glaucoma Cause?
Most open-angle glaucoma has few or no symptoms early on. As glaucoma progresses, patients may notice:
Loss of side vision
Trouble seeing objects off to the side
Missing areas in vision
Difficulty adjusting in dim lighting
Tunnel vision in advanced cases
A less common type, called angle-closure glaucoma, can happen more suddenly and may be an emergency. Symptoms may include severe eye pain, headache, nausea, redness, blurred vision, and halos around lights. Sudden symptoms like these should be evaluated urgently.
How Eye Doctors Check for Glaucoma
A glaucoma evaluation is more than just an eye pressure check. Your eye doctor looks at several pieces of information to decide whether glaucoma is present or whether you are at risk.
1. Eye Pressure Testing
Eye pressure, also called intraocular pressure, is measured during the exam. High eye pressure can increase the risk of glaucoma, but pressure alone does not diagnose glaucoma. Some patients have high pressure without optic nerve damage, while others can have glaucoma with normal pressure.
2. Optic Nerve Evaluation
Your eye doctor examines the optic nerve for signs of glaucoma damage. This may include looking at the size, shape, and appearance of the optic nerve and checking for changes over time.
3. OCT Imaging
OCT is an advanced scan that measures the thickness of the retinal nerve fiber layer and optic nerve tissue. This helps detect early structural changes that may not be obvious during a routine exam.
4. Visual Field Testing
A visual field test checks side vision and looks for blind spots caused by glaucoma. This test is important because glaucoma often affects peripheral vision before central vision.
5. Corneal Thickness Measurement
Corneal thickness can affect eye pressure readings and glaucoma risk assessment. A thinner cornea may make eye pressure readings look lower than they truly are and can be an additional risk factor.
6. Drainage Angle Evaluation
Your eye doctor may evaluate the drainage angle of the eye to determine whether the angle is open, narrow, or at risk of closure. This is especially important for patients who may be at risk for angle-closure glaucoma.
Can Glaucoma Be Treated?
Glaucoma cannot usually be “cured,” but it can often be managed. The goal of treatment is to lower eye pressure and reduce the risk of further optic nerve damage.
Treatment may include:
Prescription eye drops
Laser treatment
Oral medication in some cases
Glaucoma surgery for more advanced or uncontrolled cases
The right treatment depends on the type of glaucoma, the level of eye pressure, the amount of optic nerve damage, and whether the condition is progressing.
